Clients are accepted based on several factors, including your due date, where you live, and our availability. Submitting an intake form does not guarantee a spot in our practice.

Our midwives have hospital privileges at Oak Valley Health - Markham Stouffville Hospital and attend hospital births only at this location.

Midwives provide care for low-risk pregnancies and have a defined scope of practice. The information you provide in this questionnaire helps us determine whether midwifery care is appropriate for your pregnancy.

Providing false or misleading information to obtain care is unacceptable and will result in your application being deemed ineligible for care with our practice.
Please allow up to 14 business days to receive a response. All communication will be sent by email only.

If you are accepted into care, you will receive an email with your intake appointment details. You will have 3 business days to confirm your appointment if a spot is available.
