What Happens After You Submit Your Intake Form?

 

1. Watch for Our Email
Please allow up to 14 business days for us to review your intake form and respond by email. All communication regarding your application will be sent from a no-reply email address, so be sure to check your inbox and junk/spam folder regularly.

 

2. If You're Accepted Into Care
If we are able to offer you care, you will receive an email with your intake appointment details. You must click the "Confirm Appointment" button within 3 business days to accept your appointment. If you do not confirm within this timeframe, your appointment may be cancelled and offered to another client.

3. Reconfirm Your Appointment
Approximately 2 weeks before your intake appointment, you will receive another email with 3 required online forms. These forms must be completed at least 1 week before your appointment. Completing these forms confirms that you will be attending your intake appointment and allows us to prepare for your visit and reduces the number of no shows.

 

If the required forms are not completed by the deadline, your intake appointment will be cancelled and your spot in care may be offered to another client.

 

4. If We Are Fully Booked
If we are unable to accept clients for your due date, we will notify you by email and automatically place you on our waiting list. If a space becomes available, we will contact you.
